WVP has also created several peer support programs staffed with Registered Traditional Healthcare Workers to provide wrap around community based support for patients in IPA member practices.
WVP peer programs have been providing critical access and support for the vulnerable patients in our community since 2008 and have realized significant success in this way of delivering collaborative care.
WVP Peer Programs are collaborating with local community first responders to provide peer recovery support for any community member seeking support in taking that first step to recovery. The WVP Peers assist community members to locate chemical dependency services and support them through enrollment or admission.
MOMs + MOMs Plus
Peer Mentor Program
MOMS is the Marion & Polk Obstetric Mentoring Service. The MOMS traditional health care workers help pregnant women who struggle with substance use disorders navigate the health, recovery and social service system.

Peer Mentor Program
The Emergency Department Intervention Team traditional health care workers assist in reducing unnecessary Emergency room visits and improving use of the health care continuum and location of social resources.

Peer Mentor Program
The Family Support Coordination traditional health care workers wrap community supports around families of children with complex health conditions and assist with the navigation of health, social and school systems.
DADS Peer Mentor Program
The DADS, Dad’s Advocate Development Services traditional health care workers support fathers with a current or past history of substance use disorder to achieve and maintain sobriety and be an active part of their children’s’ lives.
RAMP Peer Mentor Program
The Resource Advocacy Mentor Program (RAMP) is an equitable and inclusive peer mentor program for community members struggling with substance use disorder to provide health and social system navigation.
