About
The WVP Clinical Support Providers is the result of a grant proposal to Willamette Valley Community Health CCO written by WVP Health Authority in conjunction with 15 small WVP IPA and NWIP Primary Care Clinics.
The goal of CSP is to increase access to adjunct service providers that smaller clinics may not have the funds to hire on full time.
Additionally, integrated access to these services supports smaller clinics to meet the CPC+ and/or PCPCH measures.
WVP-CSP consist of:
- a clinical pharmacist
- a licensed clinical social worker
- Registered Dietitian
- a quality improvement coordinator with A Traditional Healthcare Worker
Integration is critical to ensure practice collaboration and improved patient care. WVP understands that, and therefore the providers for the CSP deliver services in your office or the patients’ home. This allows for a dynamic communication flow and higher likelihood of patient follow through.

Please provide in the referral:
- Current demographics (Include: parent/guardian contact for minor children)
- Description of the requested referral: pharmacist review, diabetes education, nutrition assessment, LCSW/behavioral health)
- Pertinent chart notes
- Labs
- Medication list/supplements
- PHQ-9 for LCSW
- Transportation or language interpreter needs
