About
WVP developed the Clinical Support Provider Program in 2017 to deliver specialty services in a Primary Care office with a focus on supporting the providers goal of offering quality care in a patient-centric manner.
The Clinical Support Provider Program is staffed with a broad array of specialty providers and available for direct patient care or provider consultation
Licensed Clinical Social Worker (LCSW)
With almost 30 years of experience working with individuals 6 years and older, WVP's LCSW specializes in mood disorders, trauma, stress, and adjustment disorders. He is available for short-term counseling and refers to a long-term counselor if necessary.
Traditional Health Worker (THW)
WVP's THWs have years of experience supporting individuals with complex medical, behavioral health and multiple social determinants of health needs. They assist patients with health and social system navigation to address barriers to care and improve the overall health of the patient.
Clinical Pharmacist
The clinical pharmacist can sit with patients for a comprehensive medication review and provide education and support on medications, usage, and adherence. They will make recommendations to the referring provider for improved medication therapy options as an integral part of the patient's team, supporting best clinical practices and up-to-date clinical research for complex medication regimens.

- Current demographics (Include: parent/guardian contact for minor children)
- Description of the requested referral: pharmacist review, diabetes education, nutrition assessment, LCSW/behavioral health)
- Pertinent chart notes
- Labs
- Medication list/supplements
- PHQ-9 for LCSW
- Transportation or language interpreter needs
